Your first appointment at the Community Hope Center is an initial assessment that consists of basic questions to learn more about your current situation and ways that the Community Hope Center can help you with whatever situation you are facing. During your first meeting, you will work with an assigned Family Advocate to build a monthly case plan that you feel is achievable before your next follow-up appointment. Appointments are scheduled monthly while you are working towards self-sufficiency. You may receive food and toiletries as needed during your scheduled appointment time. You are not required to bring any documentation to your first appointment, but the Community Hope Center may refer you to other agencies that will require specific documentation for their programs.
